How to fill out the waiver online:

2. Fill out all the required information.
3. Confirm your waiver via an email.

Valid for players of all ages
Parents can fill out multiple waivers for minors on a single form along with their own waiver
Saves time at the field allowing you to get on the field faster

*It is recommended for all players to have an online waiver filled out as these waivers are valid for 1 full calendar year.
